I went a kind of in-between route - sub-fascial...it's not really under the muscle, but more supported and covered than it would be with the traditional "over the muscle" procedure. I had very little pain during my recovery and was back to training heavy around 4 weeks post-op.
To answer OP's question, if you're above the muscle and you feel like you're healed and your surgeon has given you the green llight, I see no reason why you can't be training heavy again 4-6 weeks post op. Just make sure to ease into it!
