I know it's been done to death. Just want your opinion on this. Should I run 100mcg of T4 along side my 4 iu/day of Gh?

I assume you are looking to add lean mass? Are you running test as well?

I'm 45, looking for health benefits and to lean up. I take 200mg of Delatestryl a week. Have enough gh to do 4iu/day for six months or so. Gh serum scored well.

The only reason to take T4 concurrently with HGH is if you were looking to gain mass. As HGH increase the conversion of T4 to T3 it is presumed that increasing T4 levels will allow HGH to work more efficiently...that is the theory in a nutshell.
If you are looking for health benefits and to lean out then T4 is definitely not going to help as it is inactive thyroid and requires conversion to active thyroid to be of any use fat loss wise. I would not recommend T3 ither unless you were looking to diet for a contest..otherwise it is overkill and completely unnecessary.
200Mg a week delatestryl (test enanthate) and 1-2iu 5 days per week is all you need for health benefits and to burn fat....that is of course if you are on a very strict diet and doiing low intensity cardio at least 4-5 days per week.

Clean diet, cardio 6x/week. Why gh only 5 days thanks. And you think I should do 2 iu for a year instead of 4iu for 6 months?

I suggested only 5 days because you aren't looking for mass gains and fat loss and health benefits are easily attainable 5 days per week at the same time you are saving money and extending the term.
Again, for health benefits and fat loss the longer you run the better and 2iu is plenty. Yes 4iu will do a nice job but you are doubling cost and cutting overall duration. You have to plan your strategy according to what you are trying to accomplish.

It has always been my understanding that increased GH levels cause an accelerated conversion of t4-t3 which over time will lead to a t4 deficiency. For this reason it is normally recommended to supplement a very low dose (around 25mcg) of t4 during extended GH use to maintain normal levels. Since t4 converts to t3 at around 4:1, it typically takes a large dose of t4 to see fat burning effects.

I was also curious as to using T4 to combat fatigue. Last run I was super tired all the time. Is this caused by T4 deficienty?

That is a theory by Anthony Roberts...the theory is that increasing T4 will accelerate the anabolic process. However it does not state that T4 becomes deficient and this is not the case see from blood work and TSH as well as T4 levels during HGH usage. It is one thing to cause a deficiency and yet another to add T4 to accelerate or increase an already working process.
Taking T4 with HGH for fat burning is a waste of time and money. If you are say on a precontest diet then running HGH along with T3 will greatly accelerate fat loss.
