If you get a piece really cored out by a large gauge needle, it's pretty much impossible to draw that up with a smaller needle. And even if you happened to draw that chunk onto the end of the needle, it'd get scraped off as the needle came out from the stopper.

Generally speaking, if you have any particulate in your gear, that's the lab's fault. It might result in an infection, or it might result in a sterile abscess, or it might just get kicked by your immune system. All depends...

Very hard to happen but if it was sterile, it'll be treated as a foreign object in your body and get a sterile abcess. Will be uncomfortable. But still, for something like that to happen, it'll be very hard.
